Findings: The facility failed to provide appropriate services according to the Alleged Victim’s (AV) needs for two persons assist which resulted in injury. On or about September 08, 2022, AV was transferred by Hoyer, using inappropriate sling for AVs size. AVs left leg was hit at the Hoyer sling bar, resulting in pain. AV was sent to emergency room and was found to have a fracture in the left knee. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
