- Findings
- Alleged Perpetrator 2 (AP2) failed to perform the activities described in Alleged Victim's (AV) care plan. An investigation determined that AP2 neglected AV by not providing AV with a 2-person assist during toileting which resulted in physical harm to AV. AP2's actions is considered neglect of care which constitutes abuse. The facility failed to provide a safe environment for AV which is a violation of Oregon Administrative Rules.
