Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about September 10, 2022, AV, and Witness 1 (W1) were involved in a resident-to-resident altercation in which W1 was the initial aggressor. As a result of the altercation, AV sustained a black eye and a fracture nose. The facility failed to provide a safe environment, which is a violation of resident rights is neglect of care and constitutes abuse.
