Findings: Alleged Victim (AV) relies on the facility for his/her care. On or about September 11, 2022, AV, and Witness 1 (W1) were involved in a resident-to-resident altercation in which W1 caused three (3) puncture wounds to AV’s arm that required first aid care. W1 had been on alert for behaviors in the days prior and just the day before had initiated a physical altercation with another resident that resulted in serious injury. The facility failed to provide a safe environment, which is a violation of resident rights is neglect of care and constitutes abuse.
Sanction: RCFCP23-00739 $375.00 fine assessed
