Findings: The Alleged Victim (AV) had a history of falls, resulting in injury. on or about October 4, 2022, staff noted discoloration to AV's right arm and the right side of AV's forehead. The facility failed to properly care plan in response to AV's continued bruising. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
