- According to the documentation, the facility failed to address Witness #1’s (W1) behavior of inappropriate interactions with the Alleged Victim (AV). The failure resulted in multiple inappropriate interactions between W1 and AV, causing emotional harm to AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
