Findings: The facility failed to provide a safe medication administration system to ensure the Alleged Victim (AV) received his/her medication as ordered. AV was sent to the hospital on September 16, 2022 and returned the same day, however, staff did not know AV had returned. AV went without needed medication for three medication passes, placing AV at risk for harm. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
