Findings: The facility failed to follow the Alleged Victim’s (AV) care plan, which requires supervision while ambulating due to a high fall risk. On or about September 18, 2022, facility staff were unaware of AV’s whereabouts and AV fell while ambulating outside alone in the dark, breaking his/her hip. The failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
