Findings: Witness 3 has a history of being aggressive towards staff, Alleged Victim (AV) and other residents. On or about September 20, 2022, AV and W3 were involved in an altercation resulting in W3 being on top of AV in AV’s bed. W3 had his/her hands around AV’s neck and AV was being chocked. As a result, AV was sent to the hospital and was found to have sustained marks and bruising on his/her neck and body. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP23-01097 $500.00 fine assessed
