Findings: On or about September 20, 2022, Witness #1 (W1) was having aggressive behaviors. W1 grabbed the Alleged Victim (AV) from behind while he/she was in his/her wheelchair, squeezing his/her arms and rolling AV away in his/her wheelchair. AV was not injured, however, he/she was upset and was in unreasonable discomfort. the facility's failure to provide a safe environment is a violation of resident rights, is considered neglect of care and constitutes abuse.
