Findings: The Alleged Victim (AV) has a history of injury and non-injury falls. AV had approximately six (6) falls from June 1, 2022, to September 27, 2022. On or about August 26, 2022, AV pushed h/h pendent for assistance without response. AV was found on the floor shaken and upset. The facility failed to appropriately care plan and implement reasonable interventions to address AV’s increasing and ongoing falls, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: ALFCP23-00119 $500.00 fine assessed
