- Findings
- On or about September 23, 2022, Alleged Victim (AV) was given (7) units of metabolic medication when his/her metabolic markers were not in the range to receive medication. The facility failed to provide a safe medication administration system which is a violation of Oregon Administrative Rules. Alleged Perpetrator 2 (AP2) allegedly neglected AV. An investigation determined no abuse occurred by AP2.
