Findings: The facility failed to implement interventions and appropriately care plan regarding Alleged Victim’s (AV) known fall risk. The Alleged Victim (AV) had approximately 12 (12) documented falls between December 21, 2021, and September 29, 2022, some resulting in injury including but not limited to a head injury, skin tears, abrasions, and bruising. Some interventions put in place after the falls were already documented as previously implemented. The facility failed to implement interventions to mitigate AV’s increasing fall risk, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
