- Findings
- The facility failed to provide appropriate assessment, implement interventions, and appropriately care plan related to the Alleged Victim’s (AV) repeated falls. The failure resulted in the AV experiencing three falls, the third fall resulting in a hip fracture. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
