Findings: On September 25, 2022, Witness 1 (W1) wandered into Alleged Victim's (AV) room and began to repeatedly hit AV in the face and head with a closed fist. Based on facility documentation and interviews, staff were not present to redirect W1 away from AV's room and responded to the incident after hearing AV yelling for help. As a result of the September 25, 2022, AV felt pain and sustained discoloration to his/her temple and chin. The facility failed to implement appropriate interventions and appropriately monitor W1 according to his/her known behavior and prior altercations, causing unreasonable discomfort to the AV, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-01091 $500.00 fine assessed
