Findings: Witness #1 (W1) is known to have altercations and between August 1, 2022 and October 2, 2022, W1 was physically aggressive towards others at least 16 times. On or about October 2, 2022, W1 pushed the Alleged Victim (AV) to the ground, causing AV to hit his/her head, causing a goose egg sized bump and a bruise. The facility's failure to properly care plan to address W1's physical behavior is a violation of resident rights, is considered neglect of care and constitutes abuse.
