Findings: Alleged Victim (AV) has a history of having wounds on his/her buttocks. AV was left in souled depends overnight resulting in his/her bottom being super red. AV has also been found with dried feces on his/her body and his/her beds and their room has been found dirty with garbage and food stains. AV has refused housekeeping service and he/she has also refused staff assistance. AV’s service plan does not address AV’s refusal or provide interventions for staff if AV refuses care, putting AV in risk of harm. The facility failed to care plan around AV’s refusal, which is a violation of resident rights, is neglect of care and constitutes abuse.
