Findings: The facility failed to provide appropriate oversight of the Alleged Victim's (AV) need for assistance with ambulation. According to documentation, staff were encouraged to "push" AV to walk and use threats to call AV's son if s/he refused. The Alleged Perpetrator 2 (AP2) caused emotional discomfort to the AV by using profanity in his/her presence, threatened to call his/her son, and slammed AV's walker down out of frustration. AP2's actions are considered verbal/emotional abuse. The facility's failure to ensure quality services resulted in AV experiencing emotional discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
