Findings: The facility failed to provide services to the Alleged Victim (AV). On or about September 1, 2022, staff noticed that AV had a cap on one tooth that was starting to lift, causing pain to AV when he/she ate and in general, as it rubbed on his/her cheek and tongue. This went on for over a month without anyone seeking a dental appointment for AV to correct the situation. AV lost 4 pounds in 30 days. Between the dates of August 31, 2022 and October 1, 2022, AV suffered 6 unwitnessed falls with injury, two falls resulting in AV being transported to the hospital, one for stitches in his/her forehead, the other for staples to the back of the head. The facility's failure to provide services to AV is a violation of resident rights, is considered neglect of care and constitutes abuse.
