Findings: The facility failed to implement interventions and appropriately monitor the Alleged Victim’s (AV) known behaviors and recent increase of these behaviors. The facility failed to calm AV and stop AV from yelling which resulted in a physical altercation with Witness 4 (W4), causing both AV and W4 unreasonable discomfort, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
