Findings: Alleged Victim (AV) is care planned as a high fall risk resident. Between June 30th through October 8th, 2022, AV sustained approximately nine (9) falls. The facility did not implement new interventions after the falls that occurred on or about August 18, 2022, to prevent further falls from occurring. The fall that occurred on or about September 11, 2022, was noted as avoidable because staff should have walked closely with AV after he/she refused to use the wheelchair. The facility failed to care plan appropriately and implement interventions to mitigate AV’s risk of falls, which is a violation of resident rights, is neglect of care and constitutes abuse.
