Findings: Alleged Victim (AV) and Witness 1 (W1) have a history of aggressive behaviors with other residents and with each other. AV is care planned to have safety check every fifteen (15) minutes. W1 is care planned for one-on-one support or offer activities. On or about October 10, 2022, W1 and AV were involved in an altercation, W1 was hitting AV with a metal water bottle and pulled AV’s hair, resulting in a skin tear to AV’s right arm. The facility failed to appropriately adjust care plans and implement reasonable interventions to address AVs and W1s continued aggressive behaviors. The facility failed to follow AV’s care plan and provide safety checks every fifteen (15) minutes, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
