Findings: The facility failed to provide appropriate services for Alleged Victim (AV) according to his/her incontinence care needs. Based on interviews and facility documentation, the facility failed multiple times to assess and intervene with AV's incontinence care needs resulting in failure of basic care, unreasonable discomfort, and serious loss of personal dignity to AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
