Findings: Witness #1 (W1) is care planned to be redirected from others when having inappropriate behaviors towards the Alleged Victim (AV). W1 has had multiple inappropriate interactions with AV, most recently on November 18. 2022. AV is having significant emotional distress, a feeling of being harassed and in fear for their safety from W1. The facility failed to follow the care plan to redirect W1, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
