Findings: Alleged Victim (AV) is cared planned as a not at risk of elopement person. On or about October 12, 2022, a landscaper was walking into the facility and AV walked out. This has not been the only occasion in which AV has eloped. AV also eloped on or about July 15th and 21st, and August 2nd and 12th, 2022. There is no indication the care plan had been updated to add proper interventions regarding AV’s elopement history. The facility failed to care plan, which is a violation of resident rights, is neglect of care and constitutes abuse.
