Findings: Alleged Victim (AV) is care planned to have escorts to and from the dining-room for all meals. On or about October 11, 2022, AV walked away from the dining-room by h/h self. AV attempted to sit on a chair in the hallway and missed the chair, resulting in a skin tear to AV's right knee. The facility failed to follow the care plan which is a violation of resident rights is neglect of care and constitutes abuse.
Sanction: ALFCP23-00163 $500.00 fine assessed
