Findings: Alleged Victim (AV) relies on the facility for his/her care. AV is care planned to have his/her incontinence brief changed and medications provided by the facility. AV is also care planned to be resistant to care. According to an investigation there are multiple instances where AV was left in soiled in his/her briefs, resulting in AV experiencing skin breakdown and repeated unreasonable discomfort. It was also noted that AVs medications are not given consistently according to their physician order. The facility failed to follow the care plan and provide services which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
