Findings: The Alleged Victim's (AV) care plan dictates that AV will use his/her call light to summon staff to assist him/her at night when needing to use the restroom, due to a history of falling. On or about October 16, 2022, AV requested assistance from staff with the call light, however, when no staff arrived, AV got him/herself out of bed and used the restroom unassisted, placing AV at risk for harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
