Findings: The Alleged Victim (AV) is care planned to have two staff present while providing care, for the protection of AV and facility staff. During night shift, only two staff are scheduled, and if AV needs assistance and one staff is on break or lunch, there is only one staff available to answer AV's call light. An incident occurred where Alleged Perpetrator #2 (AP2) had to answer AV's call light alone, failing to follow AV's care plan. The facility failed to have staffing at an appropriate number in order to follow residents care plans, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
