Findings: The facility failed to provide a safe environment. Witness 1 (W1) has a history of behaviors and resident-to-resident altercations, including roommates. Between September 27, 2022, and October 14, 2022, W1 was exhibiting increased behaviors. On or about October 7, 2022, the facility moved the Alleged Victim (AV) into W1’s room while work was being done in AV’s room. On or about October 15, 2022, W1 grabbed AV by the wrist and hit AV, accusing AV of stealing W1’s personal items, which resulted in AV experiencing unreasonable discomfort. The facility’s failure is a violation of resident rights, is neglect of care and constitutes abuse.
