Findings: On or about October 24, 2022, the Alleged Victim (AV) was found outside the facility at a neighboring apartment complex. AV was care planned to be on 1 to 1 care, due to his/her wandering. The facility failed to have enough staff to complete 1 to 1 care this day and AV left the facility, placing AV at risk for harm. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
