Findings: Witness #4 (W4) is known to have sexually inappropriate behaviors to both staff and residents. W4's care plan addresses inappropriate behaviors with staff, however, it does not address the behavior with other residents. The Alleged Victim (AV) had an interaction with W4 when he/she entered AV's room as asked to see AV's body parts. AV is afraid of W4 and felt sexually harassed by W4 after this incident. The facility failed to properly care plan regarding W4's behaviors towards other residents, causing AV emotional harm and a loss of dignity. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
