Findings: The facility failed to implement interventions and appropriately care plan related to The Alleged Victim’s (AV) known behaviors and recent increase of altercations. The failure resulted in AV having three known altercations with another resident with the third altercation causing serious bodily harm to the resident. The resident was then sent to the hospital for treatment. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
