Findings: The Alleged Victim (AV) has history of exit seeking behaviors. On or about October 19, 2022, a TSP was put into place; engage AV with tasks, walk and provide conversation, provide a calm quiet space, and administer PRN medication. AV is documented to exit seek multiple times after TSP, threaten self-harm behavior and have aggressive behavior towards staff. On or about October 24, 2022, AV climbed out of a facility window and attempted to climb over the courtyard fence, when staff intervened. Later that day AV climbed out h/h apartment window and walked to the front door of the facility to let staff know s/he was going home. The facility failed to provide a safe environment, implement new interventions, and have an appropriate safety plan following AV's continued exit seeking behaviors and demonstrated ability to exit the facility, which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
