Findings: Alleged Victim (AV) has a history of altercations with Witness 1 (W1) and other residents. On or about October 29, 2022, AV wandered into W1’s room and started to touch things and walk around. W1 grabbed AV by the throat and choked AV leaving redness on AV’s throat/neck. The facility failed to provide a homelike environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
