Findings: The Alleged Victim (AV) has a history of falls. AV is unable to ambulate without a wheelchair. AV has a history trying to get up from his/her wheelchair and/or bed by him/herself. On or about October 14, 2022, AV tried to get out of bed on his/her own and fell, sustaining a bruise to his/her back. The facility failed to have interventions in place regarding known behaviors of AV. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP22-01610 $250.00 fine assessed
