Findings: The Alleged Victim (AV) has history of elopement, Self-harm, and behavioral health episodes. On or about November 01, 2022, AV exited the facility behind a visitor leaving the facility. AV walked to the highway where s/he attempted to walk into oncoming traffic while making suicidal statements. The facility failed to provide a safe environment, appropriately care plan, and have interventions in place around AV's behaviors which is a violation of resident’s rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00284 $375.00 fine assessed
