Findings: Witness 1 (W1) has history of behaviors associated with Alleged Victim (AV)’s interaction with another resident. On or about November 03, 2022, W1 hit AV in the head three (3) times with h/h fist, resulting in AV becoming upset and crying. A short while later W1 hit AV in the back of the head with a plastic toy. The facility failed to properly plan care and provide adequate interventions to mitigate W1 behaviors, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
