Findings: The Alleged Victim (AV) was a known fall risk and suffered multiple falls between August and October 2022. The facility's documentation does not show any interventions put into place to alleviate AV's falls. Many of AV's falls resulted in skin injuries or head injuries. The facility's failure to properly care plan regarding falls is a violation of resident rights, is considered neglect of care and constitutes abuse.
