Findings: Witness #1 (W1) and Alleged Victim (AV) have history of sexually inappropriate behaviors with other residents and each other. W1 and AV have interventions in place regarding their inappropriate behaviors that include redirection and supervision. On or about November 04, 2022, documentation indicates W1, and AV engaged in inappropriate sexual behaviors with each other. The facility failed to appropriately adjust care plans and implement reasonable interventions to address W1s and AVs continued inappropriate behaviors with each other, which is a violation of resident rights, is considered neglect of care and constitutes abuse.
