Findings: The Alleged Victim (AV) was known to self-propel in his/her wheelchair and run into things. Between June 2022 and November 2022, AV was found to have multiple bruises and skin tears. On or about July 2022, staff were to apply geri sleeves to AV, but it was known that AV was taking them off. During this time, no investigations into the causes of the bruising and skin tears were completed and the only interventions were geri sleeves and wrapping parts of AV's wheelchair. The facility's failure to ensure that AV was safe during self-propel in his/her wheelchair is a violation of resident rights, is considered neglect of care and constitutes abuse.
