Findings: The Alleged Victim (AV) was care planned as an elopement risk. The facility failed to provide a safe environment. On or about November 7, 2022, a resident notified staff about a chair next to the fence in the outdoor courtyard. AV was discovered missing and gone for approximately forty minutes before being located walking on a nearby street attempting to hitchhike, leaving AV at risk for serious harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse. The allegation that Alleged Perpetrator 2 (AP2) physically abused AV while getting AV in the car, was investigated and the determination was not substantiated.
