Findings: Witness #1 (W1) has a history of resident-to-resident altercations and acting as though s/he is going to “ram” other residents with h/her wheelchair. On or about November 07, 2022, W1 approached Alleged Victim (AV) aggressively; AV attempted to avoid W1 by walking faster and sideways. AV fell, hitting h/h head and sustaining a laceration to h/h forehead and an abrasion to h/h elbow. The facility failed to properly plan care and provided interventions appropriate to mitigate W1 continued aggressive behaviors towards other residents which is a violation of resident rights, is considered neglect of care and constitutes abuse.
Sanction: RCFCP23-00080 $375.00 fine assessed
