Findings: On or about November 8 and November 9, 2022, Witness #1 (W1) shoved the Alleged Victim (AV). Interventions were put into place for staff to redirect residents away from W1, however, there were not enough staff available to ensure this intervention was effective. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
