Findings: The Alleged Victim (AV) was known to have a history of anorexia and was care planned for monthly weight checks. The facility failed to follow the care plan and provide appropriate services related to nutrition. Between April 20, 2022, and October 19, 2022, AV experienced an approximate weight loss of 15.2% and according to AV’s PCP, placing AV at risk for serious harm. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
