Findings: Alleged Victim (AV) was determined to be an exit seeker; however, his/her service plan did not address AV’s exit seeking. On or about November 13, 2022, the facility fire alarm was activated causing exit doors to unlock. AV was accounted for once the fire alarm was deactivated and fire doors were confirmed to be locked. At 11:00am, AV was found to be missing from the unit. AV was located approximately one (1) mile from the facility. The facility failed to care plan around AV’s exit seeking behaviors, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: RCFCP23-00418 $375.00 fine assessed
