Findings: On or about October 19, 2022, the Alleged Perpetrator 2 (AP2) popped the Alleged Victim’s (AV) narcotic medication, however, according to documentation, AP2 did not log or initial AV’s narcotics book, resulting in the narcotic medications count being off. AP2’s actions are a violation of resident rights, are considered neglect of care and constitute financial abuse. The facility failed provide a safe medication administration system, which is a violation of Oregon Administrative Rules.
