Findings: The Alleged Victim (AV) was known to fall and has had at least 15 falls prior to this most recent fall. AV suffered a fall on November 11, 2022, suffering a fracture to his/her nose. The facility failed to properly care plan to decrease AV's amount of falls. The facility failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
