Findings: Witness 1 (W1) is known to have aggressive, disruptive behaviors. On or about November 17, 2022, W1 was using their wheelchair and rammed into Alleged Victim (AV) who was also in a wheelchair. W1 grabbed AV’s right arm and wrist using both of their hands and bent AV’s arm back causing bruising and a scratch on AV's hand that was bleeding. The facility failed to provide a safe environment for AV, which is a violation of resident rights, is neglect of care and constitutes abuse.
Sanction: ALFCP23-00445 $375.00 fine assessed
