Findings: On or about November 18, 2022, the Alleged Victim (AV) was found on the side of the highway by passers by. Emergency services were called and local police department notified the facility of the possibility of finding a resident by the side of the highway. Facility staff identified AV at the location. AV left the locked memory care without the knowledge of any staff. AV was gone approximately one hour prior to having been found by police. AV was sent to the emergency department and treated for mild hypothermia. The facility's failure is a violation of resident rights, is considered neglect of care and constitutes abuse.
